Since this is your first time, there are a few housekeeping items to take care of.


Medical History Forms & COVID-19 Liability Release Forms

If you have access to a printer, please print these out and bring them to your first appointment. Click HERE to get them. If you have any problems downloading or printing them, we will have copies in the office.

Cancellations, No Shows, and Late Arrivals

Your time is precious...as is ours. We try to be as accommodating and flexible as possible because life happens. Here is our current policy on cancellations, no-shows, late arrivals, and reschedules.


Cancellations

If you need to cancel or reschedule your appointment please give at least 24 hours notice. You can call or text 508-982-8433 or email us at blackbirdblessings3@gmail.com

If you reschedule, your 50% deposit that was made to hold the appointment will carry over to your rescheduled appointment. If you cancel completely outside of the 24 hour window, we will refund your deposit.

Illness: Keeping our clients and us safe is our number one priority. If you feel unwell (fever, sniffles, sore throat, stomach issues, etc), believe you might be getting sick, or have recently been caring for a family member that is sick, please reschedule your appointment. You will not be charged even if it's within 24 hours.

Snow: If you do not feel it is safe to drive due to weather, please let us know as soon as possible.


No-Shows

If you have an appointment scheduled and you do not arrive and do not contact us, we will retain the 50% deposit used to hold the appointment.

Late Arrivals

Please arrive 5-10 minutes prior to your scheduled appointment time so we can discuss your needs for that appointment. If you are late for your appointment, we can not go over your scheduled time.
